DPT/DT 3 doses 3, 4, 5 months	Diphtheria	- Bacterial infection that affects membranes of nose and throat - Sore throat, fever, swollen glands, weakness, thick gray coating on airway - Damage heart, kidney, nervous system. Incubation: 2-5 days
DPT/DT Booster 18-mth	Pertussis (Whooping cough)	- Respiratory disease - By bacterium Bordetella pertussis - Incubation: 5-10 days
DT Booster 6-7 years 11-12 years	Tetanus	- Bacterial disease that affects nervous system, leading to painful muscle contractions, jaw and neck muscles - Commonly found in soil, dust and manure, rusty nails - Incubation: 8 days to several months
